This presentation explains how rapid review within an Integrated Care Hub provides timely access to care and diverts patients out of emergency departments and inpatient beds.

Western Health is one of Melbourne’s most prominent public hospitals in Victoria, Australia. The Integrated Care Hub is a multidisciplinary outpatient hub that provides specialist assessment and intervention by multidisciplinary teams within Rapid Access Clinics, both face-to-face and via telehealth. Patients are referred directly from the Emergency Department to prevent avoidable admissions, from inpatient settings to support early discharges and from the community. The Hub has established clinics for several specialties, including Nephrology and Endocrinology. A dedicated Aboriginal and Torres Strait Islander Clinic aims to improve access, tailored care and attendance to outpatient appointments for Indigenous people.

Kirsty Barnes (Bachelor of Physiotherapy, MBA) – Divisional Director

Kirsty is the Divisional Director of Chronic and Complex Care at Western Health in Victoria, Australia. Kirsty is an experienced clinician, researcher and leader with over 30 years of experience fostering relationships and collaboration in the community and integrated care sector. She is passionate about providing the very best in patient care, translational research and promoting innovation in clinical practice. She has an excellent collaboration track record in public and private health services settings. Her current interests are in value-based healthcare utilizing e-health to improve chronic disease management and integrated care delivery models.

Dr. Tessa Johnson (DPT, BSc) – Transition Project Director

Tessa is the Transition Project Director for Custodial Health at Western Health, Melbourne, Australia. She completed her Doctor of Physiotherapy at the University of Melbourne and has a project management background. Tessa is passionate about public healthcare delivery, fostering innovative solutions and improving patient care.
