Keynote Presentation: Building a Relational Foundation, Through Radical Collaboration, Connection, and Community: The Journey of IFIC Canada

It is well-established that healthcare systems worldwide are interested in improving population health, well-being and the integration of care locally as a key transformative lever. Although health and social care systems are working to implement integrated care, many are at varying stages of maturation, adoption, and success.

To support the advancement of integrated care and meaningful partnership with patients and caregivers, the International Foundation of Integrated Care (IFIC) Canada has created a space and brought together a community that spans lenses, disciplines, organizations, sectors, and health and community systems.

IFIC Canada is a platform and connector within the integrated care movement in Canada and abroad. IFIC Canada demonstrates the power within distributed leadership and working in partnership to mobilize knowledge and advance the implementation and evaluation of integrated care. The keynote address will highlight the IFIC Canada journey, and the capabilities and lessons learned that contribute to the communities' growth, impact, and success.

Presenter Bios:

Jodeme Goldhar

Jodeme’s passion is convening opportunities, building capabilities, and inspiring those in health and social systems to work together to realize the potential of the collective. Jodeme catalyzes large-scale transformative change and ensures change opportunities are co-designed. Listening to patients, caregivers, and those that work in health and social care is the key to creating the cultures that enable high-performing integrated healthcare systems.

Jodeme brings over 25 years of experience, collaborates locally and internationally, and is highly skilled in the most emergent methods for building and sustaining culture and capabilities to support transformation within health and social care systems.

Dr. Walter Wodchis, PhD

Walter Wodchis, PhD, is a Professor at the Institute of Health Policy, Management and Evaluation at the University of Toronto, Research Chair in Implementation and Evaluation Science and Senior Scientist at the Institute for Better Health (IBH), Trillium Health Partners, and Senior Adjunct Scientist at ICES. He is also co-lead for the International Foundation for Integrated Care (IFIC) Canada. An internationally recognized scientist in health economics, financing, health care policy evaluation and integrated care, Walter is leading research to understand and improve health system integration and performance. Dr. Wodchis has developed research to understand the needs and care trajectories for individuals with high care costs and complex health needs, particularly those with multiple chronic conditions. He has led multiple local, national and international research programs to understand the implementation of integrated care programs to address those needs. At IBH, Wodchis is working to advance a Learning Health System. As principal investigator for the Health System Performance Network, Walter also leads a team of scientists and trainees responsible for provincial evaluations of integrated care programs in Ontario.
